Pacific Consortium Created
Establishment of a small consortium of Pacific provider partners with a shared commitment to improving health and wellbeing outcomes for South Auckland communities, establishing the foundations for future collective action.
2020 - 2022
Communities leading through Crisis
During the COVID-19 pandemic, providers united around a village approach to deliver coordinated health and social support, demonstrating the power of community-led solutions to reach and care for those most in need.
February 2022
A Vision for System Change (Health Reform proposal submitted)
Building on lessons from the pandemic, partners proposed a new model of coordinated care designed to address the interconnected health, social, and economic challenges facing South Auckland whānau.
April 2022
Chosen to Shape a New Way of Working
Ōtara-Papatoetoe was selected as one of Aotearoa's first locality prototypes, creating an opportunity for communities, providers, and partners to design health solutions together.
May 2022
A Community-Led Movement Begins
Community members, iwi, mana whenua, health providers, and system partners came together to launch the Ōtara-Papatoetoe Locality and begin a shared journey toward better wellbeing outcomes.

Listening to the Community
More than 20,000 community insights helped identify local priorities, strengths, and aspirations, ensuring future investments and actions reflected what mattered most to whānau.
August 2022
Collective Commitment to Change
Twenty-one organisations signed the Ōtara-Papatoetoe Charter, committing to collective action and community-centred change.
March 2023
Community Priorities Become a Roadmap for Action
Health and wellbeing priorities were identified through community conversations and formally endorsed, creating a roadmap for action driven by local needs.
2023 – 2024
Turning Community Voice into Action
Partners worked alongside iwi and community leaders to refine a locality plan focused on delivering meaningful and sustainable improvements for whānau.
June 2024
Le Afio'aga O Aotearoa is Established
Recognising that community aspirations extended beyond healthcare alone, Le Afio'aga O Aotearoa was formed as a charitable trust to champion broader community priorities and long-term collective impact.
July 2024
Expanding Impact Beyond Aotearoa
Le Afio'aga O Aotearoa was selected to lead the New Zealand–Samoa Long-Term Twinning Placement Programme, strengthening primary healthcare capability through international partnership and workforce development.
September 2024
Locality Plan Approved
Years of collaboration culminated in the approval of the Ōtara-Papatoetoe Locality Plan, enabling implementation of community-driven priorities.
October 2024 – March 2025
Delivering Change Together
Partners implemented initiatives across Child and Maternity Care, Mental Health and Addictions, Community Outreach, and Workforce Development, demonstrating the power of coordinated collective action.
April 2025
Celebrating Collective Impact
A Collective Summit brought partners together to share achievements, lessons learned, and the positive outcomes created through collaboration across the locality.

Le Afio'aga O Aotearoa continues to bring together community, iwi, provider, and government partners to strengthen collaboration, champion community priorities, and develop new initiatives that create lasting impact across Auckland and beyond.
